Tuberculosis test

at the Municipal Health Service (GGD)

In order to obtain a residence permit for study purposes you must undergo a tuberculosis test (x-ray of your lungs).  You need an appointment for screening on Tuberculosis for the IND (Immigration and Naturalization Service).

Please fill out the form to request an appointment. You will receive the appointment date and time by e-mail.  You can also call the GGD to make an appointment. This is possible on Monday, Tuesday and Thursday between 13:00-16:00. Tel: +31 (0)88 0031 310.

The test costs are approximately €50,- (Pin only, no cash). You can declare the test to the health care insurance company afterwards. 

Please take the following documents with you:

·      Your passport

·      Your insurance pass

·      The “TB test referral form” from the IND (see VisaCare)

Please complete part 1 of the form before you go to the IND:

1.9 - Fill in the address where you live in the Netherlands (the postcode is four numbers and two letters).

1.12 - Fill in the personal details if you have a spouse or (registered) partner.

Part 2 of the form you do not need to fill in. The doctor will fill it in after you have taken the test and will send the form to the IND (Dutch Immigration Service).

·      The  two “Questionnaires Tuberculosis examination” (see below, to print them and take them to the appointment)

Please complete the forms and answer all the questions before you go to the IND:

You do not have to fill in your BSN (citizen service number).

You can find your V-number in part 1.3 on the “TB test referral form.
